Notice of Availability: Forms 1095-B and 1095-C
Form 1095-B, Health Coverage
Form 1095-B provides information about group health coverage that you were enrolled in during the year. OneBridge prepares and files Form 1095-B on behalf of reporting entities for which OneBridge provides certain health reimbursement arrangement (HRA) plan recordkeeping and administration services. Your coverage may be reported on Form 1095-C instead of Form 1095-B (see below).
How to obtain your Form 1095-B:
- If you have a YourWay portal account, you may log in to your account at portal.yourwaybenefits.com and download your Form 1095-B under the Account Activity section of your portal (most recent year made available by January 31st).
- If you do not have a YourWay portal account, you can create an account at portal.yourwaybenefits.com and then download your Form 1095-B.
- You may also request a copy of your Form 1095-B by:

- Calling us at 888-865-1628
- Emailing us at taxreporting@onebridgebenefits.com
The form will be provided to you by mail or electronically (by request) within 30 days of the date your request is received.

Form 1095-C, Employer-Provided Health Insurance Offer and Coverage
OneBridge Benefits prepares and files individual forms 1095-C on behalf of applicable large employers that sponsor a YourWay Individual Coverage Health Reimbursement Arrangement (ICHRA) for which OneBridge provides recordkeeping and plan administration services. Form 1095-C provides information about the YourWay ICHRA coverage that your employer offered you during the year.
How to obtain your Form 1095-C:
- If you have a YourWay portal account, you may log in to your account at portal.yourwaybenefits.com and download your Form 1095-C under the Account Activity section of your portal (most recent year made available by February 28th).
- If you do not have a YourWay portal account, you can create an account at portal.yourwaybenefits.com and then download your Form 1095-C.
- You may also request a copy of your Form 1095-C by:
